Specialist End of Life Care
Bryn Yr Haul is a 36 bedded Care Home providing both General Nursing and Residential Care for service users of mixed gender aged 65 years and above. Bryn Yr Haul has an excellent reputation within the surrounding area for it’s relaxed atmosphere and one where visitors are welcomed with genuine warmth.
The homes original building dating back to 1840 has a Grade 2 Listing with the extension being added in 2002.
Nestled in a quiet rural area of Mold, located between the Clwydian Mountains and the Plains of Cheshire. Mold boasts both, places of interest and natural beauty including Logger Heads National Park and Moel Famau walking trails, a little further afield is the historic towns of Wrexham and Chester.
We are a specialist provider in that we are accredited with the “Six Steps” certificate as part of the National Service Framework for End of Life Care. We have a wealth of knowledge training and experience within our staff group in meeting the physical, psychological, social and spiritual needs of people who require our service. We pride ourselves in not only supporting service users, but also their family and friends to enable them to spend quality time together.
As part of the service we are also able to offer through our dedicated team support including:
Monitoring of medical conditions through our qualified nurses- Provide support in all medication areas
- Support future wishes through our End of Life Care ethos
- Support Continence needs
- Support with personal care
- Support with Diet and Nutrition
- Physical exercise and assistance with mobility
- Monitoring of skin care, providing specialist equipment.
Service users and or their appointed representatives are encouraged to participate in developing their care plan around their personal and individual needs, together with their wishes and aspirations for the future. Meeting regularly with their Primary Nurse to discuss their care, the opportunity to also have a Key Worker appointed is available to assist them in the transition period from hospital, or their own home to Bryn Yr Haul. We also hold resident’s meetings monthly and discuss all aspects of their Home Life, all residents are welcome to attend.
